What you need:

Team information Sheet -fully completed with all contact numbers, request off dates. Please return to office by February 2, 2012 .
Turn in MYSA Membership forms for each player and each coach. Each player new to league will need a state issued birth certificate. Each coach new to league will need a copy of his coach’s license.
The cost for player cards is $19 per player and $19 per coach. Cards are good for Fall 2011 and Spring 2012 season. Dual cards will be $19. Teams that card through Raytown Soccer, but play elsewhere, cards will be $24.
